Prep for Exams

Plan to arrive 15 minutes prior to your scheduled appointment to allow time to fill out paperwork.
Help expedite your visit by having copy of your signed Referral form  if you want us to file claims with your insurance company. Copy of your report will be sent to your referring doctor.

What to bring

• Signed referral form/or Faxed to: 866-611-1704 (if your insurance is paying)
• Current and active insurance card (if insured)
• Payment in form of cash, check or credit card (for self pay)
• Co-pay in the form of cash, check or credit card (if required by your insurance company)
  

Preparation for EndoPAT Exam

• Prior to the study fast for at least 4 hour and absent from tobacco use, caffeine, vitamins and any medication that may affect vascular tune during the study for at least 8 hours
• Ware fit loose cloth with no jewelry on the upper extremities
